Forms & Waivers
Before arriving at the kayak shop please ensure that everyone in your group has completed the required forms and waivers.
Tours, Lessons and Rentals
Each adult participant must complete a Liability Waiver.
If the participant is under 19 years old, a parent or guardian must complete an Acknowledgement of Risk form on their behalf.
Summer Camp & Youth Skills
The parent or guardian of each participant must complete the Acknowledgement of Risk form and the Summer Camp Medical Form before the first day of camp. If you wish your child to be included in the photographs we take, please complete the optional Youth Media Consent Form.
Overnight Trips
If you are going on a guided overnight trip with us, please complete a Liability Waiver and a Medical Form.
Completing the Media Consent Form is optional.
